Overview 2008-09 Education Grant Winners

COMMUNITY PROGRAMS GRANT APPLICATION

The Junior League of Clearwater-Dunedin (JLCD) is once again accepting applications for its annual Community Service Program Grants. JLCD grants are provided to fund programs to be completed not later than April 1, 2011, and provide funding ranging from $500 to$1000. Eligible organizations must be 501(c) (3) not for profit organizations and provide services which serve to improve the well-being of foster youth in upper Pinellas County (north of Ulmerton Road) and Pasco Counties in keeping with the current JLCD focus area Community and A.N.G.E.L.S.: Alliance to Nurture Grow and Enhance Life Skills.

Applications are reviewed by the JLCD’s Community Advisory Board based on the information provided (see application instructions below). Applications should be emailed to jlclwdun@aol.com or mailed to:

The deadline for receipt of applications is Monday, February 22 at NOON. Applications received after this date will not be considered. Please note: Application deadline is the date of receipt, not date of postmark. Applicants wishing to mail their applications should allow sufficient time for postal service. Applicants will be notified by the end of March. Applicants receiving awards should plan to attend a check presentation at 6:30, Monday April 12.

COMMUNITY PROGRAMS GRANT APPLICATION INSTRUCTIONS

Please provide the following proposal information (no more than three pages).

1. Mailing and contact information of your organization.

2. Brief description of your organization and key members.

a) Mission
b) Background
c) Executive director and brief bio
d) Number of active staff
e) Board members – please list names and affiliations
f) Number of volunteers; please list volunteer affiliations
g) Present activities
h) Funding sources
• Current and past fiscal year funding sources
• Expected funding sources for next fiscal year
• Past Junior League support or funding, including Education Grants. If applicable, provide purpose, date, and amount.

3. Description of specific project or program for which funding is being requested.

a) Background of the program
• When established
• Purpose
• Past accomplishments
b) Vision for the program
• Program goals and how they address the JLCD focus to serve to improve the well-being if foster youth in upper Pinellas County (north of Ulmerton Road) and Pasco Counties in keeping with the current JLCD focus area Community and A.N.G.E.L.S.: Alliance to Nurture Grow and Enhance Life Skills
• Description of how program incorporates “best practices” to ensure success
• Evaluation procedures
c) Funding requirements
• Total amount of funding requested from the JLCD (maximum $1000)
• Program budget and uses of funds
• Other prospective funding sources

 In addition to the Application (limit 3 pages) please also include a copy of IRS tax-exempt certificate.

 Applications MUST be RECEIVED no later than NOON Monday, February 22
